Austin Insulin Recovery Initiative
Advancing a safe, regulated framework through which eligible unopened and unexpired insulin could eventually be recovered instead of destroyed.
Purpose
Many people receive packages containing multiple insulin pens, cartridges, or vials. When treatment changes or ends, some completely unopened insulin may remain. Today, that safe, unopened insulin is destroyed. The Insulin Recovery Initiative seeks to create a regulated framework through which it could eventually be recovered.
Where leftover insulin comes from
Gestational diabetes is the clearest example. It affects roughly one in ten pregnancies in Canada, and for patients who need insulin, that need typically ends the day their baby is born. A treatment that lasted weeks can leave behind whole unopened boxes of pens, purchased at full price mid-pregnancy, now destined for destruction. Multiply that across the thousands of gestational diabetes cases every year and the waste is substantial.
Pregnancy is only one path. Doses change, treatments switch, and sometimes a person passes away with a full package in the fridge. The program would not be limited to any one situation. Read more about the human side of this on The Barriers page.
What research shows about insulin waste
No one has properly measured how much safe, unopened insulin is discarded from Canadian homes. That evidence gap is itself part of the problem this initiative wants to fix. What has been measured points in one direction:
- A review in the Canadian Journal of Diabetes reported that a study of five Ontario hospitals found roughly a third of vial insulin was wasted, mostly due to expiration.
- A US community hospital study found that unused insulin pens had to be destroyed at patient discharge because of regulatory constraints, exactly the kind of rule this initiative wants re-examined.
- In England, researchers found that switching from disposable to reusable insulin pens would save the NHS money and cut plastic waste.
- Medication waste in general is expensive: a Canadian evidence review cites estimates exceeding $110 million per year nationally, and a recent hospital study showed recycling returned medications is financially viable.
Why insulin can’t simply be returned today
Insulin that has left a pharmacy generally cannot be returned to inventory and dispensed to another patient, because there is no reliable way to prove that it was:
- Continuously stored within the required temperature range.
- Never frozen or overheated.
- Not tampered with or substituted.
- Stored safely throughout its entire life.
- Traceable by lot and expiry date.
Visual inspection alone is insufficient.
Proposed long-term framework
A future regulated recovery system could combine safeguards such as:
- Tamper-evident packaging and serialized product tracking.
- Temperature-monitoring indicators activated when the insulin is originally dispensed.
- Digital cold-chain tracking with lot, expiry, and recall verification.
- Pharmacy inspection, acceptance, and refrigerated quarantine before approval.
- Clear rejection and destruction procedures.
- Redistribution only through licensed pharmacies or hospitals.
- A valid prescription for every recipient.
- Defined liability and quality-assurance standards.
- Government-authorized pilot testing.
The temperature monitor must begin tracking when the insulin is first dispensed. Attaching a monitor only at return would not prove its earlier storage history.
This initiative is currently in the research, advocacy, stakeholder-consultation, and policy-development stage. The Austin Project does not currently accept returned insulin. Regulators, pharmacists, and researchers interested in this work are invited to get in touch.
